On the same day, an Italian man became the first person in the world to test positive for COVID-19, monkeypox, and HIV.
According to a case study in the Journal of Infection, a 36-year-old man returned from Spain nine days prior and began experiencing a fever, sore throat, and headaches. It is unclear if he had protected or unprotected sex while on his trip; however, it's been suggested that his symptoms may be due to sexual contact with other men.
The man was admitted to the hospital and tested positive for COVID-19, monkeypox, and HIV. He is currently being treated with antiretroviral therapy and is in stable condition.
The man first got diagnosed with COVID-19 on July 2. Subsequently, within a few hours, he started developing rashes and blisters around his legs, face, body, and buttocks. The blisters were painful to the touch. He went to the ER where he was tested positive for COVID-19 again as well as monkeypox. The man is believed to have contracted monkeypox through sexual contact with another man who was infected with the virus.
He discovered he had HIV on the same day, July 6, as well. Despite being undetectable when he last tested in September, he learned that he was infected on the same day.
“Because this is the first reported case of monkeypox virus, SARS-CoV-2 and HIV co-infection, there isn't enough evidence to show that this combination may exacerbate [a] patient's condition.”
"In this case, we have a lot more proof to show that sexual intercourse might be the main way of transmission," one of the researchers stated.
It is therefore recommended that sexually transmitted infection screening be carried out after a monkeypox diagnosis.
"The findings from this case also suggest that people with monkeypox should be quarantined for at least 21 days and given supportive care," the researchers added.
The patient tested positive for HIV-1, revealing that the infection was most likely recent due to his high CD4 count. They also noted that he had previously contracted syphilis.
Given the rapid spread of monkeypox, with over 45,000 cases reported in 98 nations so far, more will certainly follow. They went on to remark that "the majority of cases were recorded in gay or bisexual men who frequently had additional infections."
The researchers explained how, as these pathogens spread, individuals can contract monkeypox virus and SARS-CoV-2 at the same time. This would obviously make it more complicated for doctors to give an accurate diagnosis because all three share similar symptoms.
What's more, the study authors suggested that the man's weakened immune system allowed him to contract all three viruses simultaneously. They wrote, "The patient's high viral loads and CD4 cell count are likely indicative of a strong pathologic response to infection, rather than severe immunosuppression."
"In the event of an epidemic, healthcare systems must be aware of this possibility and promote diagnostic testing in high-risk individuals, which is critical to prevention since there is no approved therapy," they added.
The findings were published in the journal Emerging Infectious Diseases.
